What To Do In The Event Of A Death

1. Contact Cooney Funeral Home 847-685-1002 or 773-588-5850

   • Inform the funeral director of the following:

   • Who you are (relationship to the deceased)

   • Your telephone number

   • Who has passed away

   • Location of where the death occurred

   • Make an appointment with Cooney Funeral Home and we will make immediate   arrangements for the removal

2. Gather information needed for the Death Certificate

   • Full name of deceased

   • Date of birth

   • Place of birth

   • Social Security Number

   • Level of education

   • Military discharge papers and records

   • Marital Status

   • Father’s name and Mother’s name and maiden name

3. Choose the clothing and personal items for the deceased. Clothing should include undergarments and socks. Personal items can include watches, jewelry, eyeglasses etc.

4. Gather any information that will help Cooney Funeral Home compose the death notice for the newspaper.

5. Locate any insurance policies and bring them to the appointment.

6. Decide on the number of certified death certificates will be needed for all of your financial and legal needs.
